TMSAA New Member Request Form

Thank you for your interest in joining the Tennessee Middle School Athletic Association. The information you submit via this form will assist us in creating a correct and complete record of your membership. Please be as thorough as possible.

Membership in the Tennessee Middle School Athletic Association is open to any school in Tennessee, both public and non-public, that meets the following criteria:

1. Terminates with the 6th, 7th, or 8th grade.

2. Is approved by the State Department of Education, State Department of Education approved agencies (Schools must be in Category 1, 2 or 3), AdvanceEd, and/or Southern Association of Independent Schools.

In addition to the completion of this form, we require the payment of your school's first year membership dues ($100) and catastrophic insurance fee ($200) before your membership becomes effective.
