Anyone interested in adopting a current or future rescue animal must fill this form out before they can be considered as potential adopters. Camp Cocoa reserves the right to decide whether a potential adopter is suitable for any particular rescue animal and deny that adoption if it is not a good fit.
This form is not a formal application, just an initial screening of our potential applicants.
For some of the foster animals, Camp Cocoa partners with Critter Calvary Rescue out of Tennessee. CCR assists in all the initial vetting of the animals, spay/neuter, microchips as well as transport up to CT. They also handle the formal applications, vet reference checks and personal reference checks.
