Please review the waivers below. These waivers apply to all FNL dates attended from November 2025 through January 2026 and any PYL practices that are attended during the 2026 season for trial purposes.
Minor
Waiver and Release of Liability
In consideration of being allowed to participate in any way in the Petaluma
Youth Lacrosse athletics/sports programs, and related events and activities,
including travel to and from these activities, I agree to the following:
1. Acknowledge and fully understand that each participant will be engaging in
activities that involve risk of catastrophic injury, including permanent
disability, paralysis and even death, as well as severe social and economic
losses which might result not only from their own actions, inactions, or
negligence, but the action, inaction or negligence of others, the rules of
play, or the condition of the premises or of any equipment used.
2. Assume all the foregoing risks and accept personal responsibility for the
damages following such injury, permanent disability or death.
3. Release, waive, discharge, and covenant not to sue Petaluma Youth Lacrosse,
its affiliated clubs, their respective administrators, directors, agents,
coaches, and other employees and volunteers of the organization, other
participants, sponsoring agencies, sponsors, advertisers, and if applicable,
owners and leasers of the premises used to conduct the event, all of which are
hereinafter referred to as lessees, from any and all liability to each of the
undersigned, his or her heirs and next of kin for any and all claims, demands,
losses or damages on account of injury, including death or damage to property,
caused or alleged to be caused in whole or in part by the negligence of the
leases or otherwise.
4. Agree that the parent(s) and/or legal guardian(s) will instruct the minor
participant that prior to participating, he or she should inspect the
facilities and equipment to be used, and if the participant believes anything
is unsafe, he or she should immediately advise his or her coach or supervisor
of such conditions and refuse to participate.
Permission
to Authorize Medical Care
I give authority to Petaluma Youth Lacrosse, its coaches and assistant coaches
to obtain appropriate medical attention for my child in the event my child is
injured while participating in the sport of lacrosse and in the event that I or
another parent or guardian cannot readily be contacted to grant such
permission.
Internet and Media Publishing Consent and Waiver
I understand that this consent and waiver gives Petaluma Youth Lacrosse (PYL)
permission to publish the above named participants photograph and/or name
and/or accomplishments on the Petaluma Youth Lacrosse website, PYL affiliated
sites or any media outlet.
I understand that information and/or photographs published on the Internet, or
used in newspapers, magazines, and other media sources, may be viewed by anyone
around the world. I release the Petaluma Youth Lacrosse League organization
from any liability resulting from or connected with the publication of this
information.
This permission will stay in effect until cancelled by the parent, guardian, or
participant reaches 18 years of age.
