READ BEFORE SIGNING Name of Event: 2026 Milwaukee Dragon Festival - Date of Event and Practices : July 6th - July 11th, 2026.
In consideration of being allowed to participate in any way in the Milwaukee Dragon Festival, related events and activities including, without limitation, the in Event and training or practices for the Event, the undersigned acknowledges, understands, and agrees that: 1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce this risk, the risk of serious injury does exist; and, Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others and assume full responsibility for my participation; and, 3. I willingly agree to comply with the stated Event rules, instructions from Event officials and customary terms and conditions for participation. I understand I may be removed from the Event at any time at the discretion of an Event official. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest Event official immediately. If I am provided a wristband or other credential to access the Event, I agree that the wristband or credential is issued solely for my own personal access to the Event, that I will not share, transfer, sell, lend, or otherwise provide it to any other person, and that I will use it only for my own access to the Event; and, 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS the Milwaukee Dragon Festival, Milwaukee Chinese Community Center(MCCC), NAAAP, OCA-Wisconsini, Wisconsin Chinese Chamber of Commerce (WCCC), Lakeshore Park, Milwaukee, the State of Wisconsin, Milwaukee World Festival Inc. (MWF), Pan American Dragon Boat Association, LLC, all volunteers, sponsors and all related relevant parties, the owners(s) and lessee(s) of the premises, waterways and grounds where the Event will be held, the owners, lessors, lessees of all equipment used in connection with the Event. Sponsors, sponsoring agencies, advertisers and beneficiaries of the Event and the respective departments, officers, officials, directors, members, contractors, agents, employees, licensees, invitees, servants and volunteers of the foregoing (“Releasees”),
with respect to any and all injury, disability, death or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise.
Photo Disclaimer
During my visit, I or members of my party may be filmed, videotaped, and/or photographed by a festival volunteer, contract photographer, or the media. My attendance at the festival serves as permission for the use of my image, or the image of my family members, by the festival.
Photo/Video Release Agreement
I, hereby grant Milwaukee Chinese Community Center (MCCC) and the Milwaukee Dragon Boat Festival permission to use my name, image, likeness, voice, and, if applicable, the name, image, likeness and voice of my minor child in a photograph, videos, recordings, or other media, without payment or any other compensation. These photographs, videos, recordings, or other media may be used for promotional and marketing purposes through a variety of mediums including, but not limited to, printed collateral, television media, internet, website and social media (including the MCCC Facebook, Instagram, LinkedIn, and Twitter pages, among others).
