Chaperone Interest Form - 6th Grade Rock Eagle 2026
All parent chaperones must be a legal guardian of a student attending the field trip.  Please complete this form by October 16.  We will notify all parents who complete this form by Monday October 19 if they are selected to be a chaperone.  The chaperone fee is $5.  Please do NOT pay this fee until we have confirmed you as a chaperone. Parents may choose to ride the charter bus or provide their own transportation to and from Rock Eagle.
