Cocts Akihabaraにおきましては、未成年者の方(16~17歳)のみのご宿泊の場合、保護者様に当同意書のご提出をお願いしております。
チェックインまでに、ご宿泊者様全員のフォームのご回答にご協力をお願いいたします。
*16歳未満の方は、同意書の提出があっても、保護者(18歳以上)の方の同伴がない限りご宿泊をお受けでき兼ねますので、予めご了承ください。

Cocts Akihabara requires that parents/guardians submit the consent form when only minors (16~17 years old) are staying at the hostel.
We ask for the cooperation of all guests in completing the form before check-in.
*Please note that persons under 16 years of age cannot be accepted to stay unless accompanied by a parent・guardian (18 years of age or older), even if they have submitted a consent form.
