Accordingly, I attest and affirm that the information I have provided is absolutely true and correct. I authorize the organization to contact any person or entity listed on the Volunteer application form and I further authorize any such person or entity to provide Cultivate Sports with information, opinions, and impressions relating to my background or qualifications. I voluntarily release the organization and any such person or entity listed on the Volunteer application form from the liability involving the communication of information relating to my background or qualification. I understand that Cultivate Sports will be conducting a background check on me prior to any opportunity working with children.I further Authorize the organization to conduct a criminal background investigation if such a check is deemed necessary. I have carefully read the policy and procedures of the organization, and agree to abide by them and to protect the health and safety of the children or youth assigned to my care or supervision at all times. *
I grant permission to Cultivate Sports to use photographs or electronic media images of which i am included in any presentation of any and all kind whatsoever. I understand that I may revoke this authorization at any time by notifying the organization in writing. The revocation will not affect any actions taken before the receipt of this written notification. Images will be stored in a secure location and only authorized staff will have access to them. They will be kept as long as they are relevant and after that time destroyed or archived. *
