Program Membership Application
Applied Self-Direction supports states with offering self-directed services to individuals who need Long-Term Services and Supports. Our Technical Assistance team has more than two decades of experience developing, implementing, and expanding self-direction.

At Applied Self-Direction, our mission is to promote self-direction so individuals have the choice and control to live the lives they want.
Membership is a program membership, not an individual membership. Program Membership is open only to state agencies that administer or seek to administer a self-directed program. Distinct agencies within the same state may apply separately. Program member dues rates are valid as of July 1st. Open program membership (Tier 1) has no cost. Membership begins immediately upon approval of your application and lasts through June 30th. The dues structure is subject to change on an annual basis. Open program membership (Tier 1) includes two (2) technical assistance hours. Program members who need additional technical assistance may sign-up for Tier 2-8 at any time during the membership year. Technical assistance hours not used by June 30th will expire. Program members may withdraw from membership at any point during the year, however dues are nonrefundable. Program membership dues (Tiers 2-8) must be received within 30 days of invoice. If payment is not received after 30 days, the applicant will be considered an open program member until payment is received.
Please allow approximately 1 week to process your application. The two agency representatives will be notified via e-mail once your membership has been accepted. For Tier 2-8 applicants, the billing contact will also be notified.
