Official High School Transcript Request Form
Use this form to request a copy of your students official high school transcript. Students must have received high school credit to request official transcripts. Transcripts are notarized with the official Connections Academy seal and signed by a school official. 

Current seniors must submit requests for transcripts through Naviance. For information on how to request transcripts in Naviance contact your high school guidance counselor.
                                                                                                                                   


This email address MUST match the parent or student email address that is on file with TCAH before records will be sent to personal addresses. All requests will be processed in the order in which they are received.
