Two Proofs of Residency (recent lease/rental/mortgage agreement/bill and a current utility bill or property tax statement)
Immunization Records
Withdrawal paperwork from previous school
Unofficial transcript or report card from previous school
Any legal documents related to child custody, restrictions, or restraining orders
Once you have submitted all the above required paperwork, please call 574-254-2851 for a registration appointment.
