This form is intended to register players who will be playing in the South East Ball Hockey League for the 2024/2025 Season.

All Games will be held at a location of choice by the League Executive.

There is no charge to register to play in the South East Ball Hockey League,

Please answer all questions truthfully, The responses from this form will be held securely for the period of the 2024/2025 season and will be destroyed after the conclusion of the league to abide with GDPR policy.

Please be aware that your club manager or the league registrar may contact you about your registration.

Any Players under the age of 18 will also need an additional U18 consent & waiver form completed by a parent or Guardian.
