Fill out this form to request a Grace Quarter from the Membership Committee. You will need a grace quarter if you no longer fulfill the academic status requirements as prescribed in rule II.A.2 & II.A.3 of the rulebook*. The Grace Quarter program is designed for members transitioning out of the Co-op after completing their degree/program. If you are requesting one for a different reason please be sure to explain this in the "Explain your Situation" box below.

In order to be eligible for a grace quarter from MemCom, you must satisfy the following criteria:

1. You must have been living at UCHA for the immediately preceding three consecutive regular quarters (i.e. excluding Summer) prior to the requested Grace Quarter.

2. You must continuously be in good standing until the beginning of your Grace Quarter. This includes:
   3.1  -  Having a Room & Board balance of less than $100
   3.2  -  Having no more than 2 missed Bathroom Shifts
   3.3  -  Having no more than 8 hours of Chore Deficiency

3. You must not be subject to expulsion.

Submitting this form and fulfilling the above criteria DOES NOT guarantee a being granted a Grace Quarter. Only official communication from MemCom indicates the approval of a Grace Quarter.
