JAL Dance Enrolment 2021
For RETURNING STUDENTS, please enter mandatory fields and classes. UNCHANGED DETAILS NEED NOT BE ENTERED.

The email address should be the primary contact email address that will receive all general emails. (The invoice address can be different) It will also be used as the JAL website member's page login username. If there are multiple students within a family, it MUST be the same for each student.

For NEW families and students, please enter all fields with a **.

For subsequent students, primary email address is all that is required followed by student name, birthday, enrolled classes and final declaration.

Enrolment is assumed until the end of 2021 unless notified via email. Please refer to website for fee schedule and terms and conditions. Notification for cancellation of any lessons must be received prior to the invoicing for the next payment block to avoid being charged for cancelled classes.
