I certify that I am the child's parent or legal guardian and the child is 12 years old or will be by Labor Day 2026. By filling out this form, I give my child permission to be at Rose Tree Woods Swim Club without a responsible person or adult. I understand if my child breaks any Rules or Regulations, I will be called and will have to pick my child up immediately. 

As per the Rules and Regulations of Rose Tree Woods Swim Club, managers and lifeguards are authorized to enforce all rules and regulations. Any member who violates these rules are subject to disciplinary action, up to and including revocation of membership. Repeated infractions of the rules by a minor will result in a written letter to the guardians of the minor. Repeated violations of the rules will result in removal of all pool privileges for the remainder of the season and could result in revocation of membership with no refund of pool dues or fees. Rules and Regulations are posted at the Front Office and on www.rosetreewoods.org
