CAR RIDER FORM-YEAGER ELEMENTARY

In order to increase student safety and improve traffic flow, we ask that all students who will be picked up in the afternoon as car riders have a pre-printed car rider tag. If your child will be a car rider, please fill out the information below. The car rider tag must be displayed every day in the window of the vehicle that will be picking your child up.

Con el fin de aumentar la seguridad de los estudiantes y mejorar el flujo de tráfico, le pedimos que todos los estudiantes que serán recogidos por la tarde en automóviles tengan un pase de coche. Si su estudiante va a ser levantado por automovil, por favor llene la siguiente información. El pase de automovil debe mostrarse todos los días en la ventana del vehículo que va a recoger a su estudiante.
