Circle of Grace Program

The Archdiocese of Omaha's Circle of Grace Program is required for all students participating in Catechism programs or Youth Groups in any parish of the Archdiocese. There are 3 options for parents (which are listed on the next page), and so we need you to let us know the option you have chosen for your child(ren). This information must be completed before your child(ren) can attend any Catechism classes this year. Please note that this information must be updated every year, and every family must give an updated response even if they completed the survey in a previous year

Please choose from the following options:

I would like to have the age-appropriate Circle of Grace lesson for my child(ren) emailed to me. I will use it with my children in the way I deem appropriate as the parent. (No further action or opt-out letter will be required.)

I would like the ICC Catechism teachers to teach the 3-5 lessons. Please contact me about scheduling these classes.

I choose to opt-out of the Circle of Grace program for my child(ren). (An opt-out letter or email must be submitted.)

Submitting an Opt-Out Statement from Circle of Grace

If you are not choosing to opt-out of the Circle of Grace program, you may skip the rest of this form and submit it. Parents who are choosing to opt-out of the Circle of Grace program must submit the following in writing with a signature: Safe Environment Training was offered to your child/young person You refuse to allow your child/young person to participate in the training You received Safe Environment training materials for your child/young person.
A short email or letter is sufficient to opt-out. Presumably, if you are sending the letter in an email format (or using the form at the bottom of this page) a hand-written signature would not be required, but please sign (type) your name in the email. An opt-out letter or email must be written each year.If you choose to send a letter, please send it to:
