This form is for current and former students to request school records. Please read each question carefully before submitting. Requests will be processed in the order received. Under the Family Educational Rights and Privacy Act (FERPA), only the student, or the parent/legal guardian if the student is under 18, may request student records. CHISD will follow all FERPA guidelines when releasing records. Please allow 5 days for the request to be fulfilled. Thank you.
