2018-2019 Mentorship Program Application Form
This form is to be used to apply for the 2018/19 mentorship program. Please note that space is limited and not all applicants will receive a spot in the program. Deadline: November 20, 2018.

The program will run from January to April 2019 and you are required to attend both the opening reception and present a poster of your work at our closing reception. If you are unable to do so please contact our office at outreach@math.utoronto.ca BEFORE applying for the program.

Reference
All applicants are required to have a reference letter from at least one referee. This referee must be able to speak to your math abilities and your commitment to participate in this program. Below you will write the name and email address of your referee. You will then instruct the referee to upload their letter on the following form (please send them the exact URL):https://goo.gl/forms/rxDlRD2UeRxcZy6m1. It is up to you to ensure that you send your referee the above link so they can send us your reference letter. Failure to do so may result in your application not being considered for the program.

Personal Statement *
We require each applicant to provide us with a statement on why they are interested in this program and why they feel they would make a good candidate for the program. The statement should speak to your mathematical abilities and interest in the subject. Please include the ways that you've pursued your interest in mathematics (courses, camps, competitions, independent reading, etc.) and your potential interests for university studies. Your statement should give us a sense of who you are and your readiness for this program. Please upload your statement in a pdf or word format.
