School Sponsored Activity Parental Authorization
Pursuant to Rule 6A-10.089 (5), F.A.C., Seminole County Public Schools requires parental authorization prior to students participating in school sponsored events and activities. Each student participating in the event or activity listed below must have this form completed and submitted prior to participating. 

Description of the Event or Activity: There are NO try outs.  Everyone is welcome to participate.  All you need to do is have a parent/guardian fill out the registration form, pay the $60.00 fee on School Bucks or with cash and get a physical on the District Physical form.  REGISTRATION, FEES MUST BE TURNED IN BY THURSDAY, OCTOBER 10 AND PHYSICAL MUST BE COMPLETED BY WEDNESDAY 16th.  If you have questions, please contact Coach Singer at singerec@myscps.us

Date(s) and Time(s): Practices will start on October 20th and will be every Monday 4pm to 5pm, Wednesday 3pm to 4pm and Thursday 4pm to 5pm.

Student Supervision: Supervision will be provided by Liz Singer.  For safety purposes, students will NOT be allowed to practice out without athletic shoes.
