* Please register ONLY teens in grades 7 - 12 on this form. To register children in Pre K - 6, please see the elementary online registration form. If you have a teen in grade 6 who wishes to attend Jr. High Youth Ministry events (Awake/Jr. High Fun Nights)in additional to 6th grade faith formation, please also complete both forms.
*St. Patrick's School teens, do not need to register for the Confirmation Ministry. Your teen is registered via school enrollment unless you opt out. However, we ask that you complete this form if your teen would like to participate in the supplemental Jr. High Youth Ministry events.
* Teens must have the registration form & liability waiver turned in before attending any sessions. *After registering, you will receive a follow up email with a schedule of dates and liability waivers.
