Parental Release We recognize and acknowledge that there are risks in my child’s/ward’s presence and participation in this school sponsored program. I agree to indemnify, hold harmless, waive and relinquish all claims I may have against including any negligence claims on their part and its officers, agents, employees, representatives or volunteers arising out of, in connection with the transportation to and/or from the event, or any activity my child participates in while participating. We hereby agree and consent that you are authorized to render first aid and emergency treatment as you may deem necessary in the event that our child is injured as a result of participation in athletic sports or other school related activities and contests of Nova-Davie Baseball without the necessity of prior consultation with us. In the event of any such injuries, you are authorized to transfer our said son/daughter to such hospital as the school may deem advisable, and obtain the assistance of such physician or other assistance as the school may deem necessary. I will permit my child to be photographed, videotaped, and/or interview by camp staff for the use of camp publications and/or website broadcast. *
