SB 12 Parent Consent Forms
Effective September 1, 2025, Senate Bill 12 will REQUIRE parent consent for students to receive school-based health-related services, counseling services, and for participation in student clubs and organizations. Please complete this form for each PAISD student in your household. Failure to complete and submit this form for your child(ren) will be considered as refusal of consent. This means your child will not be seen by the school nurse and/or counselor, nor will they be allowed to participate in district or campus student clubs. Please note your consent for services or participation can be withdrawn in writing.
