By submitting this form you are stating that you understand that you can receive formal complaints that lead to Infraction points as determined by the Elysium LARP Team; that your second formal infraction in 10 games attended will place you on the public Restricted List, and being on the Restricted List bans you from using a Bow & Arrow for the next 10 games attended.

By submitting this form you agree to not let anyone who is not listed on the public Opt-In use your Bow & Arrow, you agree not to let anyone who is on the public Restricted List use you Bow & Arrow, and you agree to not intentionally* aim at or near persons on the public Opt-Out list.

*We understand that accidents happen, these instances will be judged on a case by case basis to determine intent and severity of the incident.
