Eanes ISD GT Consent for Assessment for Grades 2nd-8th
The focus of the Eanes ISD GT Program is to serve students who have instructional needs due to exceptional intellectual and creative abilities.

Assessment takes place once a year for 2nd - 8th grade in August.   (2023 - 2024 Kindergarten and 1st-grade assessments will take place in January.)

Please only complete the form once for each child you are having assessed.

Deadline for Completion/Submission: This form must be signed and submitted by AUGUST 16, 2023 (11:59 PM).
