Thank you for registering your unit for B-MaP-C Study.

Please note, as per data regulations, your details will will be used for administration of the study only, and will NOT be provided to anyone else. Once the study is complete, all your saved personal details will be deleted permanently. By completing this form, you are consenting to use of your name, institution and contact information to be used to generate REDCap access and for study communication only. This will include study updates, RedCap login details and at the end of the study, certificate of participation and a survey request.

On completion of this form you will receive an email with a welcome letter, and a separate email with details of REDCap access. Please ensure the following is completed before commencing data collection:

1. Register the audit with your audit department and email us a copy.
