This form will provide information to Carrollton City Schools should you need your child to participate in the After School Program.

PARENT AWARENESS :

Hours of operation are from 2:45 - 6:00 PM Monday through Friday.
Children must be picked up by 6:00 PM or a late fee $15 for the first 5 minutes and an additional $1.00 per minute charge will be added to the student's account.
ASP is closed for any Professional Development days, Holidays, and School Breaks.
ASP is open on Early Release Days. ( unless advised otherwise )
An after-school snack will be available for each child.
Behavior rules and procedures are to be maintained throughout the day including after school program hours. (see student handbook for behavior guidelines)
Failure to follow the rules can result in your child being expelled from the After-School Program.
If a child is expelled from the After-School Program, he or she will not be allowed to attend for the remainder of the school year.
Disrespect of staff, refusal to follow instructions, fighting, and bullying of any type are grounds for immediate dismissal.

AFTER SCHOOL PROGRAM FEES:

Daily: $12.00 - Prepaid - Revtrak (Cash Payment only as an exception) ( NO checks)
Weekly: $60.00
Late Payment Fee: $25.00* per month unpaid
After 1 weeks of non payment. The student is unable to attend until balance is paid in full
Late Pick up fee: $15 dollar for the first 5 minutes and an additional $1.00 per minute charge
Pre-payments are due Mondays
No balance will be allowed to carry over unless previous arrangement is made with the Program Director.
Any account with an outstanding past due balance at the end of the month will incur a late fee of $25.00 monthly and will result in the student being suspended from the program until the balance is paid in full. All payments for the month of May must be prepaid no exceptions.

STUDENT EXPECTATIONS:

Respect yourself and others
Avoid inappropriate language
Eat and drink in designated areas
Demonstrate positive behavior at all times
Follow the instructions of the staff
Resolve disagreements in a positive way
Running is allowed in the gym or outside recess areas only
Be respectful of other members and their property
Take care of the school facility, grounds, and equipment
If you make a mess, clean it up. Student Handbook behavior policies and consequences are upheld during After-School Program hours.
