Please fill out this form to pre-register for the Endewearde Arts and Sciences Championships.  The event will take place on August 20, 2022.  Pre-registration is not required, but is highly encouraged.  An invoice will be sent to the email address on this form.

Event Fees are as follows:
Adults (18+): $10.00 ($5.00 with proof of membership)
Youth (6-17): $3.00
Children (0-5): $0.00
