This form is to request services for disability-related accommodations for only events and activities at Western Oregon University (WOU). This includes Deaf and Hard of Hearing communication services like Sign Language Interpreting, TypeWell Transcribing, Real Time Captioning Services (CART), and ProTactile Interpreting, as well as general accommodations such as auditory related access, mobility related access, and other forms of access (note taking, support service providers, furniture, etc). Please do not fill out this form if you are a WOU student and want to request ongoing accommodations during your time as a WOU student.

Please submit this request form at least five (5) business days before the event. Requests submitted less than five (5) business days can be submitted, but accommodations are not guaranteed.

All requests must be submitted in Pacific Standard Time (PST).
