The Sports Vouchers program is a Government of South Australia initiative administered by the Office for Recreation, Sport and Racing. The program provides an opportunity for primary school aged children from Reception to Year 9 to receive up to a $200 discount on sports or dance membership/registration fees. The purpose is to increase the number of children playing organised sport or participating in dance activities by reducing cost as a barrier. For more information on the Sports Vouchers program visit

By submitting this form, you give permission to the Woodville Lacrosse Club (WLC) to share your information with the Office for Recreation, Sport and Racing (ORSR). The ORSR may share information to the Commonwealth Government for the purpose of validating the identity of a child with Medicare and with third parties (including research institutions or organisations) to improve the administration, outcomes or effectiveness of the program.
WLC will submit your details on your behalf, and deduct either $100 or $200 from your child's playing fees for 2026, depending on whether you use one or both vouchers entitled to your child for the year. More information on playing fees at

Please note, two Sports Vouchers valued at $100 each may be used per eligible child per calendar year. If two Sports Vouchers have already been claimed for your child for other activities in 2026, you will not be eligible to claim a Sports Voucher for lacrosse as well.
