How would you best treat a patient with HIV and CD4 of 350 cells/microliter who also had moderate to severe inflammatory bowel disease in the outpatient setting?

Biologic therapy should be used only if the patient requires hospitalization

Oral 5-ASA products should be used preferentially in order to spare these patients from biologics given their already compromised immune systems

Biologic therapy should be used based on what is appropriate for their underlying Crohn’s Disease or Ulcerative Colitis

Steroids should be used to treat acute flares but these patients should otherwise not receive disease modifying therapy
