2017 -2018 Ladle's Disclosure Document
Please read through carefully with your student and complete the required information at the end so I can give your student credit for completion!

I'm Ms. Ladle.  I hail from Ohio but I'm a recent graduate from Brigham Young University where I was a collegiate athlete and received my degree in Biological Science Education with a minor in Coaching.  I'm looking forward to an AWESOME year!  In addition to class work and lessons, students will have the privilege of participating in labs and other hands-on activities.  Below I've laid out my basic policies for how my classroom will operate during the school year.

ABSENT/TARDY POLICY
I understand that absences occur do to unexpected events, doctor's appointments, etc.  Since the students are no longer in elementary school - I expect them to be responsible for coming to me and asking what they missed.  I will not hunt students down if they missed a day of class.  They may also e-mail me and/or check the website to see what they're missing out on.
If a student accrues excessive tardies, a student may lose out on the privileges of participating in labs and other hands-on activities. A tardy is when a student is not ready at their desk with required materials when the final bell sounds.  (For further notes on Hawthorn's Attendance/Tardy Policy, please see : https://hawthornacademy.org/Docs/Attendance_Policy.pdf)

CLASSROOM RULES
Students will be expected to adhere to these 4 simple rules this year:
1. Raise your hand to speak.
2. Raise your hand to leave your seat.
3. Behave appropriately.
4. Be respectful to self and others.

They may share the binder with other classes.


DISCIPLINE POLICY
In case of instances where a student is not being an example to their peers and disregarding the classroom rules, the following procedure could be as follows:
*1st Disturbance: Reminder of expected behavior and verbal warning
*2nd Disturbance: Reminder of expected behavior and final verbal warning
*3rd Disturbance: Asked to briefly leave the classroom and wait in the hall, teacher will come talk with student to better understand issue or misunderstanding.
*4th Disturbance: Note or phone call home to parents.
In case of continued class disturbances, further actions may be taken.
~Lunch detentions and/or after school detentions may be given after the 2nd disturbance in class depending on the severity of classroom disturbances or student behavior.
~Behavior reflection sheets may also be used in class to help remind students of proper behavior in the classroom.
~Clean-Up Crew is another possible consequence that will be used occasionally in class where students will be required to stay only 30 seconds after class to help pick up trash/clean room/straighten desks/etc.  This consequence will be reserved for students whose behavior does not line up with respecting the classroom space (by throwing things, spilling or defacing materials, etc.)


GRADING POLICY
Students will not be able to earn an A for simply "being present".  Students must be engaged and active if they want to master the material and concepts being taught in the classroom this year.  Instead of receiving a grade on whether or not their bum was in their chair, their grade will come mostly from whether or not they learned the material.  With that being said - assessments, quizzes, tests and projects will be worth 80% of a student's grade.  Participation in laboratory experiments and classroom activities will be worth 10%.  Homework and assignments will account for the final 10%.  All assignments are due 1 week before the end of the quarter.  Extra credit will be offered on certain assignments when students go above and beyond the requirements.  Some extra credit assignments may be available at teacher discretion. Grade percentages are standardized throughout the school.

A  100 - 94%
A - 90 - 93%
B+ 87 - 89%
B   84 - 86%
B-  80 - 83%
C+ 77 - 79%
C   74 - 76%
C-  70 - 73%
D+ 67 - 69%
D   64 - 66%
D-  60 - 63%
F   59% and below

If a student cheats, copies from another student's assignment, homework or test, or copies information from the internet and claims it as their own work- all parties involved will receive a zero for the grade.  Students may or may not have the chance to make up points if they receive a zero for cheating. (This does not include copying notes if a student was absent).

TEST/QUIZ POLICY
The material this year has many concepts that build off of each other.  The final test will be cumulative and students are responsible for any and all material covered in class or on homework assignments whether or not they were in class or did the assignment.
I have a strict "No Talking Policy" during tests and quizzes.  I will be very clear as to when this period begins and ends so the students are not caught unawares that a test or quiz has started. If a student talks during a quiz or test, one verbal warning may be given - after which if that student talks again, a zero will be given for that test or quiz and the student will have to schedule a time with me to come in before or after school, or during the tutoring time on Fridays to retake it.

CELL PHONE POLICY
I will enforce the West Jordan Hawthorn Academy Cell Phone Policy which is as follows:
*Between 8:00 and 3:05 cell phones should be turned off and put into a bag or locker.  This includes lunch.  There are no exceptions.
If a cell phone is seen in my class, it could be taken and the student may collect it at the end of the day.

The Dress Code Policy can be found on the Hawthorn Academy Website (https://hawthornacademy.org/dress-code).  The Dress Code Policy for Hawthorn Academy will be strictly enforced in my classroom.  If a student is not abiding by the dress code, they will be excused from class to go change or to go to the office to call parents for a change of clothing if necessary.
