This tool was developed in order to improve the experiences of people aiming to reduce health disparities through advocacy initiatives by providing structure while developing a health equity and anti-racism oriented mindset. We ask that you are thoughtful and thorough in your responses to the following questions, as your feedback is an important aspect of the development and implementation of this tool.
