Please review the below procedures for submitting nominations:

- Each Sport Committee may make one (1) nomination.

- This form should ONLY be submitted by the Sport Committee Chair with the representative nomination (or selection) from that sport committee.

- Please review the Award Criteria to make sure your candidate meets the criteria to be considered for this award.

- All fields in this form are required fields, please be sure you have the following details known for ease of submitting your nomination:
 
   -  Nominator's Contact Information (Name, Email)
   -  Nominee's Contact Information (Name, Email, Phone Number)
   -  A brief description of the Nominee's accomplishments specific to this award.
