Makeup Service Agreement
This contract outlines an agreement and all contractual obligations made the day this form is submitted,
Allison Ming-Yan Lai, herein after referred to as Amazing Makeup You Love
- and -
herein after referred to as Client.
Client Full Name
Client agrees to a signed contract for Amazing Makeup You Love to provide makeup services on the date and time agreed.
Amazing Makeup You Love agrees to perform these services for Client under the terms and conditions set forth in this contract.
Accuracy of this form is extremely important in providing effective and timely services by the Artist in consideration of the mutual promises set forth in this contract; it is agreed by and between Client and Amazing Makeup You Love.
Relationship of Parties
Amazing Makeup You Love is not to be considered an agent or employee of Client for any purpose. The parties intend that an independent contractor relationship will be created by this contract. Client is interested only in the results to be achieved; the conduct and control of the work will lie solely with Amazing Makeup You Love.
Amazing Makeup You Love obligations
Makeup Artist shall provide Services with reasonable care and skill.
Makeup Artist shall perform the Services promptly on the date and time agreed.
If Makeup Artist is unable to perform agreed services for any unforeseeable reason or unavoidable situation, Amazing Makeup You Love cannot perform the agreed upon services due to unforeseen circumstances or any other cause beyond her control, any and all payments shall be returned to the client with the understanding that there will be no further liability with respect to the agreement.
Artist shall be the exclusive Make up Artist for the Event. (Others may be permitted to make up only at Artist’s discretion.)
Amazing Makeup You Love shall use her own products in the provision of the Services
Amazing Makeup You Love will arrange means of transport and have their own mobile phone but may claim legitimate expenses subject to prior agreement.
Makeup Artist will use good quality professional materials.
Client agrees to pay Fee in accordance with Payment provision
Client is responsible for Amazing Makeup You Love’s fees, assistant fees, parking and travel fees.
Client agrees to immediately inform Amazing Makeup You Love and its assistants or agents regarding any sensitivities or allergies they may have to any types of makeup, products, or treatments.
Client agrees to inform bridal party to inform Amazing Makeup You Love of any sensitivities or allergies they may have to any types of makeup, products, or treatments.
Makeup: $65.00 per person
False eyelashes are an additional $5.00.
Deposit: Upon receiving a deposit as per the prices below, it will be considered a booking. The Client will be paying Amazing Makeup You Love a non refundable amount, even if cancelled.
Bookings for 1-4 people = $25.00
Bookings for 5-8 people = $50.00
Bookings for over 8 people = $75.00
The remaining balance is due upon completion of services the day of the Event. All necessary travel is included in quoted prices.
Inside of the City of Winnipeg, no fee is charged. Anything outside of the Perimeter will be charged $0.50 per kilometer.
If Amazing Makeup You Love is required to travel out of Province or 2 hours out of Winnipeg area or due to an early call time before 7am extra charges will incur. Client will be responsible for hotel, travel, gas and per diem allowance for meals.
If for any reason, the date is cancelled, the deposit is forfeited.
Client agrees not to hold Amazing Makeup You Love, its agents, assistants, and staff responsible for any allergic reactions developed as a result of the make-up application that may occur to Client or bridal party.
Client agrees to allow Amazing Makeup You Love to take before and after pictures to use for promotional use. for advertising purposes on its website and other promotion materials.

This agreement represents all the terms and conditions agreed upon by the parties. No other understanding or representations, oral or otherwise, regarding the agreement shall bind any of the parties.
Upon acceptance of the booking, Amazing Makeup You Love will send an invoice via email for the required deposit. The deposit can be paid via the online payment or arrangements can be made to submit a cash or e-transfer deposit.

It is my pleasure to make this a fun and unforgettable experience for both you and your entire Event party, and of course to make you look and feel your very best! Congratulations and Best wishes!
