CSI Event Registration Form
Please make sure to fill out the form completely and CSI will email you a confirmation for your event as soon as possible. Please remember that all events have to be registered at least five class days before the event date.

Events with alcohol need to be registered at least two weeks in advance.

All events will need to use the Guidebook "Badger Campus Guide" to track attendance through QR scans. Masks must be worn at all times, 6ft social distance maintained, and COVID room caps should be upheld.
