We value your privacy! Any and all information you share with the Pitchfork Pantry via this form will remain completely private and will only be shared with relevant people associated with the Pitchfork Pantry, like the Dean of Students Office or DoorDash.
This form must be submitted by the Wednesday before the distribution at 11:59 pm and the pantry will review and let you know by the Friday before the distribution.

DoorDash charges the Pitchfork Pantry $5 for every delivery (instead of free as it was in the past).  With this, we are only able to accept a certain number of deliveries each month during our 2nd Saturday distribution.  
In order to qualify, you must live less than 15 miles from our Tempe location per DoorDash (215 E. University Dr. Tempe AZ).  
FILLING OUT THIS APPLICATION DOES NOT GUARANTEE YOU A DELIVERY.  The pantry leadership team will evaluate the applications and let you know by the Friday before the Saturday distribution.  
You will be notified if you are selected.
