The Salvation Army of Waukesha County Application for Christmas Assistance
**To be filled out once per family**

Eligible families will be either matched with a sponsoring family/organization or matched with gifts donated by the community through our Toy Program. Please remember that gifts ideas are suggestions only and that gifts will be provided according to the donations received. Please note all items will be pre-packaged and have curbside pick-up by appointment only. Once your application has been processed, you will receive a pick-up day and time. If you are deemed to be ineligible to receive gifts through the Salvation Army for any reason, you will be notified by mid-December. Please remember that you can only sign up for Christmas Assistance through one agency.
