What configuration fields do you need?
What Label, Key, and Field Type do you want? 1 row per field. Anything you're not sure of, just ask and we'll get back to you! Example: "Your Name - name - Text field"
What is your email address? (So we can contact you with any questions, and you can ask us to update this info in future)
