2026-2027 RBE Report an Absence 
-Please fill out this form WITHIN 5 DAYS of the absence.
-For all HEALTHCARE APPOINTMENTS, a DOCTOR'S NOTE IS REQUIRED WITHIN 5 DAYS upon your student's return to school.
-Late notes/notifications will not be accepted per district guidelines.
-A doctor's note is required if your student is out for more than 5 consecutive days.
Form must be filled out for each day that the student is absent from school
Please allow 48 hours for attendance updates to be made
