Biological Waste Disposal Form
Biological waste boxes ready for pick-up must be:
  • Upright
  • Tied with a single knot
  • Folded and taped shut with sturdy tape
  • Under 45 pounds
Sharps containers must fit inside of a biological waste box. Departments are responsible for purchasing sharps containers directly from a vendor.

Stericycle labels/barcodes are no longer given to laboratories and are not required for pick-up. Please write the PI name on the top of the box and ensure it is correctly entered into this form for identification and billing purposes.
