Important Notice: Important Notice: The nature of Sober Living Company requires the immediate expulsion, without prior notice or refund of any deposits, of any member found by the proprietors holding the lease on the property to: 1) be using alcohol or drugs 2) be in default of weekly house share of expenses 3) be guilty of disruptive or any behaviors that threaten the wellbeing of the property and/or the other members that reside at Sober Living Company. A member of Sober Living Company is considered a resident member of the sober living home.
I have read and understand the above notice and acknowledge that I am applying for a bed at Sober Living Company as a member of the sober housing program. I agree to abide by Sober Living Company’s principles and fully submit myself to the rules and expectations of the house, including periodic drug testing.
I understand that I am subject to immediate expulsion from the house by the proprietors holding the lease on the property if any of the following occur: 1) I use alcohol or drugs (other than prescribed medications) 2) I fail to pay my weekly house-share expenses and/or fees; or 3) I engage in disruptive behavior or any conduct that threatens the wellbeing of the property and/or the other residents of Sober Living Company.
By signing below, I certify that the information contained in this application is true and accurate. I understand and accept the conditions of membership at Sober Living Company and agree to abide by these conditions should I be accepted as a resident. Applicants and residents of Sober Living Company must be at least eighteen (18) years of age at the time of admission.
