Please complete the form below as we prepare for the 2026-2027 school year.  Please also note:
  • The Office of Pupil Transportation determines if your child is eligible to receive an OMNY Card for the bus, train or school bus based on your address.
  • Any last-minute transportation changes must be sent in writing to Ms. Berihuete in the Main Office or via email at SBerihuete@schools.nyc.gov.
