Qualifications and Requirements for Belt Testing:

*Approved if a Master/Instructor says you are READY for a BELT TEST. Minimum of 8 weeks or 16 classes taken. When you receive 3 stripes on your belt, you can apply.

Note: This form must be filled out completely and test must be paid online or in person before the testing date indicated above in order for student to be able to test for their next belt.

I hereby make application for testing at Cho’s TaeKwonDo Academy and upon acceptance I sincerely pledge to obey all the rules and regulations which are established for keeping the order of this school and for the protection of the students from injury. I hereby release Cho’s TaeKwonDo the owner, instructors, members from all repair abilities and all claims for injuries I may receive from practicing TaeKwonDo. I further concern that any pictures furnished by or taken of me may be used for promotion and I may waive compensation in regard there to. I understand that tuition fees must be paid in advance and there will be no refunds under any circumstances.
