2018/19 Distinguished Service Award
For members to nominate for the 2018/2019 Distinguished Service Award of the Year category they must complete the nomination below. All nominations will be assessed on the Award criteria.

An affiliate/region can only nominate one person for the Distinguished Service Award. No more than 12 will be awarded annually to keep in line with the prestige of the Award.

A NSWTA or TFA Life Member or Hall of Fame Member is not eligible for nomination in this category.

A distinguished Service Award can only be received once.

Nomination Period - 30th June 2018 - 1st July 2019

This nomination will be confirmed in writing after nominations close 5pm Friday 28th June
