Preferred First Name Change Request
As part of our effort to be a welcoming environment for our transgender community members, this form allows a student and/or employee to designate a preferred first name for use in some Hope College systems and communications. The last name (surname) will remain unchanged.
By submitting this form, you are requesting that Hope College update your preferred first name on non-legal documents/systems, such as in Moodle and on class lists.
This form will be submitted to the Office of Community Belonging and Title IX. You will likely hear from the Associate Dean for Community Belonging and Title IX (or designee) in order to have a discussion and/or meeting about next steps, general support on campus and to allow an opportunity to verify the request submission.
Hope reserves the right to deny a preferred name if it contains inappropriate or offensive language, or is used for the purposes of misrepresentation or fraud.
