PARENTAL CONSENT FOR AN EDUCATIONAL VISIT

CHALFONT ST GILES JUNIOR SCHOOL – YEAR 6

Year 6 Ultimate Frisbee Festival at The Chalfonts Community College, Narcot Lane, Chalfont St Peter, Buckinghamshire SL9 8TP.

On Thursday 26th January 2023 , 11.30 to 14.30 (Event time 12:00 to 14:00)

All of the children in Year 6 will be participating in a Year 6 Ultimate Frisbee Festival at Chalfont Community College on Thursday 26th January 2023 between 11.30 and 14.30 (event time is from 12.00 to 14.00). The children will be travelling to and from the festival by coach.

As children will leave for CCC at 11.30, all children will need to bring a packed lunch to eat before they go

Children should come in to school wearing their school PE kit (green T-shirt, black shorts or black jogging bottoms and green hoodie) and trainers.

Children will also need a named water bottle to take with them.

This PE Festival is organised by the Sports Partnership that is run by local secondary schools. There is no charge for this trip as membership of the Sports Partnership and coach costs to and from CCC are covered by our Government PE grant.

By completing this form I am giving consent for my child to attend the Year 6 PE Ultimate Frisbee Festival at The Chalfonts Community College on Thursday 26th January 2023. I confirm that I have read the relevant information above. Consent forms to be completed and returned by Thursday 12th January.

I agree to their participation in the activities described and I acknowledge the need for and expect them to behave responsibly. I can confirm that the school has current contact and medical details for my child.
