All individuals, groups and organizations looking to reserve a board room or the community room at the Main Library are required to complete this form. A staff member will respond within 24 business hours of receiving your completed application. ANY ENTITY CLAIMING NON-PROFIT STATUS MUST PROVIDE A TAX EXEMPT NUMBER. Condominium Associations are considered for-profit in that they function to manage private property. Room use by non-Farmington groups/residents falls under the higher fee schedule listed above, regardless of tax status. Non-profit Farmington rate requires a Farmington Library card holder to be the contact person listed above. When signed by an authorized representative, this application signifies agreement of the organization/individual to abide by the policies and regulations that govern the use of the Library’s meeting rooms, and agreement to the fee requirements outlined above. In checking the "I Agree" box below you are signing this document.
