I grant Sports Officials Care and its partners the right to photograph, record, and use my child’s name, image, likeness, and/or voice in photos, video, or other media for any lawful purpose, including publicity, marketing, website, and social media use, without compensation or further approval.

I understand that these images or recordings may be edited and used without identifying my child by name, and I release Sports Officials Care from any claims related to the use of such media, including claims of misrepresentation or invasion of privacy.

I further release and hold harmless Sports Officials Care, its partners, and their representatives from any and all claims or liabilities for injury or damages, whether physical or mental, that may occur to my child in connection with participation in the event, except to the extent caused by gross negligence or willful misconduct.
