RSU 29 2024-2025 STUDENT REGISTRATION
Complete this form to register your child/children for District RSU 29  for the UPCOMING 24-25 school year

You will need to provide to the school before starting:  Birth Certificate, Shot Record, Proof of Residence

Beginning September 1, 2021 no child is permitted to attend school without evidence of immunization or proof of immunity against disease. Students enrolled in grades PreK through 12 are to be immunized against diphtheria, tetanus, pertussis, measles, mumps, rubella, polio and varicella. Additionally, all students enrolled in grades 7-12 must be immunized against meningococcal disease. Since it takes time to complete immunization series, this is the time to call your child’s primary care provider to set up a schedule to start receiving vaccines if your child is going to be able to attend the first day of school this fall. Your school nurse can let you know what immunizations they are missing for school compliance.

NOTE:   The answer you give below will help determine what services you or your child may be eligible to receive under the McKinney-Vento Act.  Students who are protected under the ACT are entitled to immediate enrollment in school even if they do not have the documents normally needed, such as proof of residency, school records, immunization records, or birth certificate.  After the student has been enrolled, the new school must contact the last school attended to request the student’s educational records, including immunization records, and the Homeless Liaison will help the student get any other necessary documents or immunizations.  Students who are protected under the McKinney-Vento Act may also be eligible for other services.
The questions below are intended to address the McKinney-Vento Act 42 U.S.C. 11435, and must be completed for each student that registers.  The information you provide is confidential.  Your child will not be discrininated against based on the information provided.  Please check one box below regarding your or your student's housing in order to help determine services that your student may be eligible to recieve. *
