The Season Show will take place on Sunday January 25, 2026!

A parent or legal guardian must fill out this form. All forms must have a parent/guardian email address. Forms submitted without a parent/guardian email address will not be accepted.

Students rank their top 3 shows in order of preference. Students are encouraged to discuss their preferences with their instructor prior to the submission of this form.

Student preference is only one of many considerations in the placement process, and students are not guaranteed placement in their top preferences. 

Make sure to fill out your rehearsal time availability accurately!  

Only list instruments the student is enrolled in lessons for at School of Rock. While we are sometimes able to cast beyond that, we prioritize casting and show assignments based on the instrument lessons that students are actively taking with School of Rock.
